On the cover page please include the client(s) name and a brief statement that sounds something like "please release FEMA information to prevent possible duplication of benefits."
For all other states please work with your FEMA VAL (contact information varies). This is a vital step in the case management process. Completing this step ensures that VAL can provide any additional assistance or guidance on any unresolved FEMA matters, inform case managers about any additional inquiries for other agencies about the FEMA benefits for the same client, and properly advise case managers on possible appeal client may qualify for.

To do this you will want to have the client sign a FEMA ROI and in MS only fax it to our office (228-594-3222, ATTN: KIM or STEPHANIE) with a cover page including your name, organization name, best phone number to reach you at, and a fax number, if available. On the cover page please include the client(s) name and a brief statement that sounds something like "please release FEMA information to prevent possible duplication of benefits."
For all other states please work with your FEMA VAL (contact information varies). This is a vital step in the case management process. Completing this step ensures that VAL can provide any additional assistance or guidance on any unresolved FEMA matters, inform case managers about any additional inquiries for other agencies about the FEMA benefits for the same client, and properly advise case managers on possible appeal client may qualify for.
