I did not expect my remarks to be so widely dispersed. I agree that "guerilla" teams are needed. But there are patients everywhere now, a team does not have to go far to find a desperate situation. IMAT has the experience and the will to deploy available volunteers in a mission profile that makes sense. So do others. But we do not want to be involved in encouraging unqualified people to operate in environment that even military organizations would find difficult. Extended supply lines, no intelligence, poor maps, even poorer roads, no clear evacuation route. Angels fear to tread there. What we will do is assemble teams of qualified individuals desiring to operate as IMAT teams and provide them with the best mission planning and operations support we can manage. In January the first team had several former military people, and all members had extensive recent wilderness recreational and/or SAR experience. That team deployed into a largely unknown environment in Jacmel and Leogane on January 19th, with the best information and support that seven nearly sleepless days and nights by our cadre could provide them. We deployed teams into Pakistan with the same care, picking an area where American volunteers could operate relatively safely and where the need peaked about the time the teams arrived. If we get volunteers with legitimate "guerilla" qualifications I would be proud to prepare intelligence briefings for them. But I don't want to be party to sending people with out suitable background racing into the mountains. Prior to maybe two weeks ago, J/P HRO was recruiting almost exclusively for their camp and the nearby rubble clearing effort. But they have been sending "their" medics out to other areas in this emergency. They too have a pipeline and an able staff, one could do a lot worse than to volunteer with them. Chris corrected some of the things I said in my note. In particular, I did not know that one of the "solid" contacts he made in Jacmel was actually deployed there from Northern Haiti and is back in the north now. I've been tied up, as Jim has, with family stuff. MSF and the Red Cross are very difficult to work with if you have no previous relationship with them. Physicians and to some extent nurses can get into their pipeline with some effort, others (e.g. paramedics) can't at all. And they are not known for operating guerilla teams. IMAT exists, like Leslie's organization, because qualified people wanted to deploy and found there were no organizations willing to embrace them. Our volunteers take vacation time, pay their own expenses, and usually help secure donations of supplies. The medical and tactical skills of the available volunteers, and their logistical, financial, physical and emotional endurance limits determine what mission profiles we can undertake. With the funding we requested from the UN flash appeal we plan to organize specifically highly mobile multi-disciplinary teams to try to get ahead of the epidemic with a combination of outreach and treatment. Without funding we will get as close to that as possible with the resources we can muster. Cholera has burned through about 10% of the population in the last 3-4 weeks, right? I'm not a medic but it is my understanding this isn't like the flu, prior infection (even asymptomatic) does not confer complete immunity. I don't see any reason to believe this will be over any time soon. Jim, you've got the medical intelligence part of this down. If people want to self-deploy, meet up at a designated point and form teams on the spot to be picked up by suitable transport and taken to a CTC, I think that would be a wonderful thing. That's how the best of the small NGOs got started, people got on planes and went, figured out what to do when they got there. I don't want any of them wearing IMAT badges, though, unless we have proper C3I in place. I'd like to say IMAT could play a role in rapidly assembling teams at one or more jumping off points but I won't commit Chris to that without consultation. In any case I don't see how we could provide the tactical mobility needed. Look, I want to help. I don't know of another organization to send your volunteers to, if I did I'd tell you. I'm not trying to build an empire here. Our entire operation is volunteer, most of it is my brother working on his iPhone frankly. We have no funds left and no boots on the ground right now, first scheduled team of the new rotation series is due on the 6th. We do have a long-term base "safe house" and logistics arrangements. Our last team worked the night shift at the MSF CTC in Cite Soleil, keeping their patients alive through the night. I do not want to be one of those people who try to pull everything into their own organization but we do have some established processes. We also have a bank of airline miles that can be used for the Miami-PaP hop, or to help volunteers who deploy more than once. If you would like to combine forces the door is open.
